W-2 Information

If you are an employee of UC Davis you can receive your information electronically or in paper format. The fastest way to access your information will be to use the electronic retrieval method. This can be done using the At Your Service On-line (AYSO) Web site.

Electronic W-2 from AYSO
The W-2 at AYSO will be an original W-2, not a duplicate copy, and will require you sign into your account. If you have not accessed your account at AYSO previously, use the following instructions:

• Access AYSO and click on the "Sign in to My Accounts" button, next to the yellow star
• Using the New User link follow the instructions
• Enter the requested information to establish your password
• You will be prompted to agree to the electronic authorization and to choose six questions that will be asked every time you change your password
• Under the Income & Taxes heading, choose the W-2 link

Forwarded W-2 Request Form
If you are no longer an employee of UC Davis, or requested your W-2 in paper form and have moved, your W-2 can be forwarded to you at your new address once it is returned to Payroll Services by U.S. mail. Complete the request form and submit to Payroll Services using the instructions on the form.
