W-2 Information

There are several ways to access your W-2 information. If you are an employee of UC Davis, you can access your information electronically or in paper format. The fastest way to access your information will be to use one of the electronic retrieval methods. This can be done using the At Your Service On-line (AYSO) Web site; or the duplicate W-2 page via this Web site for employees that did not request electronic access.

Electronic W-2 from AYSO
The W-2 at AYSO will be an original W-2, not a duplicate copy, and will require you sign into your account. If you have not accessed your account at AYSO previously, use the following instructions:

Access AYSO and click on the "Sign in to My Accounts" button
Using the New User link follow the instructions
Enter the requested information to establish your password
You will be prompted to agree to the electronic authorization and to choose six questions that will be asked every time you change your password
Under the Income & Taxes heading, choose the W-2 link

Electronic Duplicate W-2
In order to access a duplicate copy of your W-2 between tax years 2001 and 2007 you cannot have requested electronic access to your W-2 (see above instructions for AYSO if you requested to receive your W-2 electronically). You must have:

Acrobat Reader 5.x (or better) installed on your computer
Kerberos Login and Password
Social Security Number
Date of Birth

Duplicate W-2 Request Form
If you are no longer an employee at UC Davis, or don't have access to the Internet, you can request a duplicate W-2 be mailed to you. Complete the request form and submit to Payroll.
